ABSTRACT

Objetivo: A falta de leitos hospitalares no Brasil é queixa comum entre usuários do Sistema Único de Saúde. Objetivo: Relatar a experiência da construção de um Serviço de Gerenciamento de leitos e apresentar a atuação do enfermeiro como gestor, em prol da visibilidade e fortalecimento da classe de enfermagem. Método: Relato de experiência da implementação da gestão de leitos de um hospital público estadual de médio porte, em um município do interior do estado de São Paulo. Resultado: A partir da implantação houve mudanças no perfil dos indicadores dos setores assistencias, com a utilização dos leitos aproveitados em sua capacidade máxima. Observou-se a diminuição da fila de espera para internação em consequência do acesso oportuno e ordenado à vaga. Conclusão: Pode-se inferir que o gerenciamento de leitos é efetivo e eficiente na gestão hospitalar com resultados operacionais e financeiros satisfatórios e um fator preponderante para a segurança e satisfação dos clientes.(AU)


Objective: The lack of hospital beds in Brazil is a common complaint among users of the Unified Health System. Objective: To report the experience of the construction of a Bed Management Service and to present the nurse's role as manager, for the visibility and strengthening of the nursing class. Method: Experience report of the implementation of bed management in a public hospital of medium size, in a city in the interior of the state of São Paulo. Result: From the implementation there were changes in the profile of the indicators of the care sectors, with the use of beds used to their maximum capacity. A reduction in the waiting list for hospitalization was observed as a result of the timely and orderly access to vacancies. Conclusion: It can be inferred that the management of beds is effective and efficient in hospital management with satisfactory operational and financial results and a preponderant factor for the customers' safety and satisfaction.(AU)


Objetivo: La falta de camas hospitalarias en Brasil es una queja común entre los usuarios del Sistema Único de Salud. Objetivo: Relatar la experiencia de la construcción de un Servicio de Gestión de camas y presentar la actuación de la enfermera como gestora, para la visibilidad y fortalecimiento de la clase de enfermería. Método: Relato de experiência da implementação da gestão de lechos de um hospital público estadual de médio porte, em um município do interior do estado de São Paulo. Resultado: A partir da implementação houve mudanças no perfil dos indicadores dos setores assistência, com o uso de camas utilizadas ao seu máximo de capacidade. Observou-se a diminuição da fila de espera para internação em consequência do acesso oportuno e ordenado à vaga. Conclusão: É possível inferir que a gestão de camas é eficaz e eficiente na gestão hospitalar com resultados operacionais e financeiros satisfatórios e um factor preponderante para a segurança e satisfação dos clientes.(AU)

ABSTRACT

The main objective of the study is to find the Internet awareness, Internet attitude, Internet browsing behavior among the B.Ed., Trainees in thanjavur district with their different dimensions of Age, Gender, Major subject ,Qualification ,Type of college, Gender of college ,Locality of college and Internet usage time. To analyse Internet awareness, Internet attitude, Internet browsing behavior among the B.Ed., Trainees by collecting the data from thanjavur district B.Ed., college students. For this investigation, a questionnaire developed the scholar to seek the information from the student teachers. we took 300 samples to investigate and the result shows there is no significant difference in the Internet awareness of the B.Ed., trainees and in all dimensions with respect to the variables. The study also showed a significance difference in men and women student teacher related to the Internet attitude. The Internet browsing behavior have significant influence on the B.Ed., trainees in thanjavur district.

ABSTRACT

Enterococci are Gram-positive cocci that are normal inhabitants of the urogenital and intestinal tracts of human beings and animals. Of the pathogenic species of Enterococci, Enterococcus avium is an infrequent cause of human infections. This report is an atypical case of bed-sore infection caused by E. avium in an elderly male with multiple comorbidities. Although the patient was given antibiotics along with surgical debridement, he passed away. This case calls attention to the pathogenic prospect of E. avium in clinical settings

ABSTRACT

RESUMO A pandemia trouxe vários desafios a toda estrutura social, requisitando a organização e desenvolvimento de políticas públicas para conduzir o quadro sanitário. Com relação à saúde, as ações inicialmente envolveram medidas não farmacológicas e preparação do sistema de saúde. No caso do Brasil, em março de 2020 foram deliberadas portarias específicas sobre a oferta de leitos UTI Covid-19. Em abril de 2020, o estado do Espírito Santo iniciou a estruturação dos hospitais referência para Covid-19 habilitando leitos em hospitais públicos de gestão direta e indireta, e em hospitais privados e filantrópicos. Assim sendo, o objetivo deste estudo é analisar a relação de compra e oferta de leitos exclusivos para Covid-19 pela rede SUS no estado do Espírito Santo. Os dados revelam uma rede com diferentes formatos jurídicos, com forte presença de setor filantrópico, seja por Organizações Sociais de Saúde (OSS) ou hospitais. Por fim, o estudo concluiu que a condução da rede hospitalar para leitos UTI Covid-19 ocorreu pela heterogeneidade de formatos jurídicos, com a participação da gestão pública direta substituída por diferentes tipos de gestão, condicionando o estado a reafirmar as contratualizações conforme lógica do mercado.


ABSTRACT The pandemic brought several challenges to the entire social structure, demanding the organization and development of public policies to guide the health situation. With regard to health, actions so far have involved non-pharmacological measures and preparation of the health system. In the case of Brazil, in March 2020, specific ordinances were deliberated on the offer of COVID-19 ICU beds. In April 2020, the state of Espírito Santo began structuring reference hospitals for COVID-19, enabling beds in public hospitals under direct and indirect management, and in private and philanthropic hospitals. Therefore, the objective of this study is to analyze the relationship of purchase and supply of exclusive beds for COVID-19 by the SUS network in the State of Espírito Santo. The data reveal a network with different legal formats, with a strong presence of the philanthropic sector, whether by Social Health Organizations (OSS) or hospitals. Finally, the study concluded that the conduction of the hospital network for COVID-19 ICU beds occurred due to the heterogeneity of legal formats, with the participation of direct public management replaced by different types of management, conditioning the state to reaffirm the contractualizations according to market logic.

ABSTRACT

Resumo A pesquisa investigou proposições legislativas sobre fila única de internações e leitos de UTI no âmbito do Poder Legislativo Federal no primeiro ano da pandemia. Tratou-se de estudo exploratório, qualitativo e de base documental que analisou projetos de leis sobre o tema. Os resultados foram organizados conforme o perfil dos autores e o conteúdo qualitativo dos projetos. Preponderaram parlamentares do sexo masculino, filiados a partidos de esquerda e com formação profissional em áreas que não a da saúde. A maioria das proposições tratou de fila única geral, especificando leitos hospitalares, gestão mista e indenização mediante tabela SUS. A Câmara dos Deputados apresentou mais projetos, porém pouco tem avançado em suas tramitações. Entre os projetos analisados, apenas um foi priorizado na Comissão Externa de Enfrentamento à COVID-19. Concluiu-se que o Poder Legislativo Federal perdeu, uma vez mais, oportunidade valiosa de legislar para o futuro e preparar o país com um arcabouço normativo capaz de enfrentar emergências sanitárias ainda desconhecidas, mas que exigirão muito dos gestores e do SUS.


Abstract This study investigated legislative proposals on the single waiting list for hospitalizations and ICU beds within the scope of the Federal Legislative Branch in the first year of the pandemic (2020). This was an exploratory, qualitative, and document- based study, which analyzed bills analyzed in the Brazilian National Congress on the subject. The results were organized according to the authors' profile and qualitative content of the bills. There was a predominance of male parliamentarians, affiliated with left-wing parties and professional training in areas other than health. Most bills dealt with a general single waiting list, specifying hospital beds, the mixed management of hospital beds, and indemnity through the Brazilian Unified Health System's (SUS, in Portuguese) price table. The House of Representatives presented more bills, but no progress was made in their processing. Among the analyzed bills, only one was prioritized in the External Commission to Combat COVID-19. It was concluded that the Federal Legislative Branch once again missed the chance to legislate for the future and prepare the country with a normative framework capable of confronting health emergencies, which will demand much from health managers and SUS itself.

RESUMO Objetivo: identificar as contribuições do Núcleo Interno de Regulação para a segurança do paciente. Método: pesquisa qualitativa desenvolvida entre agosto a outubro de 2020. Foram realizadas entrevistas audiogravadas junto a 13 profissionais que atuavam nas enfermarias, no pronto-socorro, na gestão da qualidade e no Núcleo Interno de Regulação. Os dados foram analisados com o auxílio do software IraMuteq® e as etapas propostas por Creswell. Resultados: os achados revelaram que o Núcleo Interno de Regulação contribui para a segurança do paciente, entornando as metas instituídas: comunicação efetiva; identificação do paciente; redução do risco de infecções associadas aos cuidados em saúde - a pandemia de COVID-19 foi apresentada como um importante dado; segurança para cirurgia, uma vez que agiliza o acesso ao hospital para procedimento cirúrgico; e diminuição de filas de espera. Ainda, contribui para prevenir complicações decorrentes de quedas, pois o paciente pode ser alocado com agilidade num leito seguro. Por fim, o enfermeiro, no seu papel de liderança do serviço e como elo para a gerência do cuidado seguro, também se mostrou importante. Conclusão: embora algumas fragilidades tenham sido detectadas, a contribuição do Núcleo Interno de Regulação se sobressai por fortalecer as metas da segurança do paciente. Em razão disso, reafirma-se a importância de fluxos regulatórios na perspectiva de gestão de leitos hospitalares, assim como os preceitos da segurança do paciente almejada pelos gestores. Não obstante, o enfermeiro atua como elo entre esses dois cenários.


RESUMEN Objetivo: identificar los aportes del Núcleo Interno Normativo para la seguridad del paciente. Método: investigación cualitativa desarrollada de agosto a octubre de 2020. Se realizaron entrevistas audiograbadas a 13 profesionales que trabajaban en las salas, en el servicio de urgencias, en la Gestión de Calidad y en el Núcleo Interno Normativo. Los datos fueron analizados con la ayuda del software IraMuteq® y los pasos propuestos por Creswell. Resultados: los hallazgos revelaron que el Núcleo Interno Normativo contribuye a la seguridad del paciente, desbordando los objetivos establecidos: comunicación eficaz; identificación del paciente; reducción del riesgo de infecciones asociadas a la asistencia sanitaria - la pandemia COVID-19 se presentó como un dato importante; en la seguridad para la cirugía, ya que agiliza el acceso al hospital para procedimientos quirúrgicos y, en la reducción de las colas de espera. También contribuye a la prevención de complicaciones derivadas de caídas, ya que el paciente puede ser ubicado rápidamente en una cama segura. Y, finalmente, el enfermero, en su papel de líder en el servicio, como enlace en la gestión del cuidado seguro, también resultó ser un resultado importante. Conclusión: aunque se detectaron algunas debilidades, se destaca la contribución del Núcleo Interno Normativo en el fortalecimiento de las metas de seguridad del paciente. Como resultado, reafirma la importancia de los flujos normativos desde la perspectiva de la gestión de camas hospitalarias, así como los preceptos de seguridad del paciente deseados por los gestores. Sin embargo, la enfermera actúa como enlace entre estos dos escenarios.

Conclusion: although some weaknesses were detected, the contribution of the Internal Regulation Center stands out for strengthening patient safety goals. As a result, the importance of regulatory flows from the perspective of hospital bed management is reaffirmed, as well as the precepts of patient safety desired by managers. Nevertheless, the nurse acts as a link between these two scenarios.

ABSTRACT

Objective:To explore the pathological differences of surgically resected specimens of advanced esophageal squamous cell carcinoma (ESCC) to different neoadjuvant therapies (neoadjuvant radiochemotherapy and toripalimab combined with neoadjuvant radiochemotherapy).Methods:Thirty patients diagnosed with advanced ESCC who underwent surgical operation after neoadjuvant therapy in Jiangsu Cancer Hospital from October 2020 to September 2021 were included. Among them, 15 patients received neoadjuvant radiochemotherapy (radiochemotherapy group) and 15 patients were treated with toripalimab combined with radiochemotherapy (immunotherapy combined with radiochemotherapy group). Surgically resected specimens were collected. The histopathological features of primary esophageal lesions and the responses of involved lymph nodes were analyzed and compared between two groups.Results:The major pathological response (MPR) rate in the radiochemotherapy group was 10/15, and 14/15 in the immunotherapy combined with radiochemotherapy group ( P=0.17). The pathological complete response (pCR) rate of the primary lesions in the radiochemotherapy group was 7/15, and 10/15 in the immunotherapy combined with radiochemotherapy group ( P=0.46). In the radiochemotherapy group, the incidence rate of tertiary lymphoid structure (TLS) was 7/15, and 12/15 in the immunotherapy combined with radiochemotherapy group ( P=0.02). The incidence rate of necrosis in the radiochemotherapy group was 6/15, and 1/15 in the immunotherapy combined with radiochemotherapy group ( P=0.03). In addition, the incidence rate of foam cell infiltration in the radiochemotherapy group was 6/15, and 13/15 in the immunotherapy combined with radiochemotherapy group ( P=0.01). Furthermore, the pCR rate of involved lymph nodes in the radiochemotherapy group was 7/33, and 11/12 in the immunotherapy combined with radiochemotherapy group ( P<0.001). Conclusion:Compared with the radiochemotherapy group, the incidence of TLS and foam cell infiltration is higher, the incidence of necrosis is lower and clinical efficacy of involved lymph nodes is higher in the immunotherapy combined with radiochemotherapy group, prompting that toripalimab combined with neoadjuvant radiochemotherapy exert higher synergistic immune effect.

ABSTRACT

Objective:To explore the improvement of diaphragm function after early off-bed mobility intervention in intensive care unit (ICU) patients undergoing mechanical ventilation.Methods:A randomized controlled trial was conducted. A total of 147 adult patients undergoing mechanical ventilation admitted to ICU of Affiliated Hospital of Zunyi Medical University from October 2019 to March 2022 were enrolled. The patients were divided into control group and observation group by convenient sampling. Except for the different intervention programs of early mobility, other treatment and nursing of the patients in the two groups were carried out according to ICU routine. Progressive early activities were performed in the control group, while early off-bed mobility was performed in the observation group. The changes of diaphragm thickness at the end of inspiratory (DTei), diaphragm thickness at the end of expiratory (DTee) and diaphragm thickening fraction (DTF) before and 24, 48, 72 and 96 hours of intervention, and the duration of mechanical ventilation, length of ICU stay and 24-hour re-intubation rate after intervention were compared between the two groups.Results:Among the 147 patients, there were 4 cases of detachment in the control group and 5 cases of detachment in the observation group. Finally, 138 patients were enrolled, 69 cases in the control group and 69 cases in the observation group. There was no significant difference in gender, age, diagnosis of ICU, sedatives, muscle strength, ventilator model, acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score and DTei, DTee, DTF before intervention between the two groups. The DTei, DTee and DTF in both groups were increased gradually with the extension of intervention time, especially in the observation group [DTei (cm) at 24, 48, 72 and 96 hours of intervention in the observation group were 0.247±0.014, 0.275±0.016, 0.300±0.013 and 0.329±0.013, while in the control group were 0.242±0.015, 0.258±0.013, 0.269±0.014, and 0.290±0.017, effect of time: F = 993.825, P = 0.000, effect of intervention: F = 82.304, P = 0.000, interaction effect between intervention and time: F = 84.457, P = 0.000; DTee (cm) of the observation group were 0.213±0.014, 0.227±0.013, 0.243±0.016, 0.264±0.010, while in the control group were 0.213±0.016, 0.218±0.013, 0.224±0.013, 0.234±0.014, effect of time: F = 385.552, P = 0.000, effect of intervention: F = 28.161, P = 0.000, interaction effect between intervention and time: F = 45.012, P = 0.000; DTF of the observation group were (15.98±4.23)%, (21.35±4.67)%, (24.09±4.44)% and (25.24±3.74)%, while in the control group were (14.17±4.66)%, (18.11±3.92)%, (20.22±4.19)% and (20.98±4.12)%, effect of time: F = 161.552, P = 0.000, effect of intervention: F = 49.224, P = 0.000, interaction effect between intervention and time: F = -4.507, P = 0.000]. The duration of mechanical ventilation and length of ICU stay in the observation group were significantly shorter than those in the control group [duration of mechanical ventilation (hours): 112.68±12.25 vs. 135.32±22.10, length of ICU stay (days): 7.84±1.78 vs. 10.23±2.43, both P < 0.01]. However, there was no significant difference in 24-hour re-intubation rate between the observation group and the control group (0% vs. 2.90%, P > 0.05). Conclusions:Both early off-bed mobility and progressive early activities can prevent diaphragm weakness in ICU patients undergoing mechanical ventilation, and the effect of early off-bed mobility is better. Early off-bed mobility can significantly shorten the duration of mechanical ventilation and length of ICU stay, and it is safe and feasible.

ABSTRACT

Objective:Severe trauma events are emergent, with low incidence and unpredictable. Current guideline does not provide precise recommendations on how the trauma centers should arrange the number of beds in trauma intensive care units while making rational use of medical resources. We analyzed the trauma intensive care unit bed requirement in the branch campus of our hospital to propose a reasonable assessment.Methods:Patients with severe trauma sent to the Intensive Care Unit of Peking University People's Hospital from January 2022 to June 2022 were collected. The daily number of patients received intensive care was counted. The bed requirement of the intensive care unit covering 99% of clinical needs was calculated based on the probability distribution function.Results:From January 2022 to June 2022, 103 patients with severe trauma [74 males and 29 females, aged (51.47±16.06) years, ranging 16 to 87 years] were included in the study. Among the 103 patients, 57 were injured in traffic accidents, 26 fell from a high altitude, 12 fell, 4 were hit by heavy objects, and 4 were stabbed. TISS ranged from 16 to 50. The range of the daily bed requirement in the intensive care unit was 0–10, which was consistent with the Poisson distribution. According to the probability distribution function, nine trauma intensive care beds could meet 99.19% of clinical needs.Conclusions:In severe traumatic events, patients need to be transferred to intensive care unit as soon as possible. For our branch campus, nine trauma intensive care beds can cover more than 99% of clinical needs. It follows that, in accordance with the basic requirements of trauma center construction, hospitals with trauma centers need at least 9 beds in intensive care units. However, traumatic events cannot be predicted; thus, the bed requirement needs to be regularly evaluated.

ABSTRACT

Retinal vascular bed area(RVBA)is the total area of retinal vasculature segmented by ultra-widefield fluorescein angiography(UWFA)images, and is an objective absolute value in square millimeter. RVBA is mainly affected by the diameter and length of retinal vessels, and whether RVBA increases or decreases depends on the “competition” between ischemia and angiogenesis, indicating subtle changes in retinal vascular morphology. As a new indicator for the study of retinal vascular diseases, RVBA may have higher stability and accuracy than the ischemia index(ISI)and non-perfusion area(NPA). RVBA is currently mainly used to evaluate the progression and prognosis of diabetic retinopathy(DR). It was found that retinal total RVBA in DR eyes was greater than the normal eyes and it decreased in DR eyes after anti-vascular endothelial growth factor(VEGF)treatment. These findings provide favorable support for the study of microvascular lesions in DR. In this article, the application of RVBA in DR was reviewed to provide a reference for the clinical study of RVBA in other retinal vascular diseases such as retinal vein occlusion(RVO).

@#Introduction: Malaria poses immerse public health challenges in the world as it is still causing significant morbidity and mortality especially in endemic regions such as Zambia. One of the effective tools in preventing malaria is the application of insecticide-treated bed nets (ITNs). This study aims to explored the socio-acceptability of malaria and ITNs among rural village community of Mazabuka, Zambia. Methods: A semi-structured questionnaire was administered to 177 respondents via two different approached methods; focus group discussions and one-to-one interviews to assess the level of KAP of the respondents about malaria and ITNs. Results: Most of the respondents (98.7%) possessed ITNs that were provided free of charge by the Ministry of Health Zambia. Overall, the knowledge and attitude of respondents on malaria and ITNs were at moderate levels of 68.1% and 71.8%, respectively. In contrast, the level of practice was poor with only 36.2% of positive response. Although 92.1% of the respondents answered correctly on the association between malaria and mosquito bites, myths and misconceptions were still common as some of them still attributed malaria to drinking dirty waters (32.8%), bad weather (15.8%), witchcraft (3.4%), and bathing dirty water (19.8%). The practice was significantly associated with knowledge (p=0.003), but not attitude (p=0.230). Logistic regression analysis revealed that respondents with high knowledge level and tertiary education were more likely to use ITNs correctly (OR=2.957; OR=21.739, respectively). Conclusion: The present study showed that the knowledge gaps were remained among the villagers as misconceptions and their believe of myths were still exist.

Objective:To improve the evaluation method of hospital beds efficiency based on diagnosis-related groups (DRG), and to provide a basis for hospitals to allocate beds reasonably and improve bed efficiency.Methods:Taking a tertiary hospital in Beijing as the research object, the types of beds were evaluated by the beds utilization matrix with the time consumption index as the X-axis and the bed utilization rate as the Y-axis. The types of beds in the department were divided into efficiency type, pressure type, turnover type, and idle type. The efficiency of medical services and the level of diagnosis and treatment were evaluated by the weight of DRG per bed. The calculation method of theoretical number of beds was improved by incorporating hospital case mix index as a risk adjustment factor into the formula to evaluate the status of beds allocation. Combining the bed type, DRG weight per bed, and bed allocation status, the improvement emphasis and management strategy of bed utilization could be comprehensively analyzed.Results:Among the 24 departments in the hospital, there were 5, 9, 1 and 9 departments being efficiency type, pressure type, turnover type and idle type, respectively. The weight per bed of 11 departments was higher than the average level of the hospital. There were 16, 5, and 3 departments with appropriate, fewer, and excessive beds, respectively.Conclusions:The comprehensive analysis of beds utilization type, allocation status and weight of each bed based on DRG is an effective method to evaluate the efficiency of hospital beds, and can provide decision-making basis for hospital bed resource allocation, hospital operation focus adjustment, and subject development planning.

Introducción: En la Facultad de Tecnología de la Salud se estudia la carrera Sistemas de Información en Salud que incluye en su plan de estudios diversas asignaturas como Sistemas de Información de Estadísticas de Salud, en la que se incluye el estudio de los indicadores hospitalarios del recurso cama; materia que se imparte de forma tradicional y no tiene una herramienta tecnológica de apoyo que facilite el proceso educativo. Objetivo: Diseñar un prototipo de multimedia educativa que favorezca el auto-aprendizaje de los indicadores del recurso cama hospitalaria en la asignatura Sistemas de Información de Estadísticas de Salud de la carrera Sistemas de Información en Salud. Método: Se realizó un estudio de desarrollo tecnológico donde fueron encuestados estudiantes y se entrevistó a la profesora de la asignatura para la obtención de la información. Se utilizó la metodología de Proceso Unificado de Desarrollo y el Lenguaje Unificado de Modelado. Se emplearon las herramientas Visual Paradigm para el modelado, Axure para la creación del prototipo y Mediator para el futuro diseño de la multimedia. Resultados: Se identificaron los problemas de aprendizaje desde la visión de alumnos y profesores, lo que permitió diseñar el prototipo de multimedia educativa sobre los indicadores hospitalarios del recurso cama, que fue considerado útil y pertinente. Conclusiones: Se diseñó un prototipo de multimedia educativa que de manera combinada con el método actual de enseñanza facilitarán y efectuarán de forma dinámica las actividades en el proceso docente educativo(AU)


Introduction: Health Information Systems career is included In the Faculty of Health Technology; the career have in its curriculum subjects as Health Statistics Information Systems, in which the study of hospital resource bed indicators is carried out; the subject is taught in a traditional way and does not have a technological support tool that facilitates the educational process. Objective: To design an educational multimedia prototype that favors self-learning of the indicators of the hospital bed resource in the Health Statistics Information Systems subject of the Health Information Systems career. Method: A survey of students and interview of teachers permitted to obtain the information needed to carry out a study of technological development using Unified Development Process methodology and Unified Modeling Language; Visual Paradigm tools were used for modeling; Axure for prototyping and Mediator for future multimedia design. Results: there were identified learning problems from the perspective of students and teachers, which allowed the design of the educational multimedia prototype on hospital bed resource indicators, considering it useful and pertinent. Conclusions: The design of the educational multimedia prototype, combined with the current teaching method, will facilitate and dynamically carry out the activities in the educational teaching process(AU)

RESUMO Objetivo: Conhecer dados sobre recusa de leitos nas unidades intensivas no Brasil, assim como avaliar o uso de sistemas de triagem pelos profissionais atuantes. Métodos: Estudo transversal do tipo survey. Com a metodologia Delphi, foi criado um questionário contemplando os objetivos do trabalho. Foram convidados médicos e enfermeiros inscritos na rede de pesquisa da Associação de Medicina Intensiva Brasileira (AMIBnet). Uma plataforma da web (SurveyMonkey®) foi a forma de aplicação do questionário. As variáveis deste trabalho foram mensuradas em categorias e expressas como proporção. Foram usados o teste do qui-quadrado ou o teste exato de Fisher, para verificar associações. O nível de significância foi de 5%. Resultados: No total, 231 profissionais responderam o questionário, representando todas as regiões do país. As unidades intensivas nacionais tinham mais de 90% de taxa de ocupação sempre ou frequentemente para 90,8% dos participantes. Dentre os participantes, 84,4% já deixaram de admitir pacientes em leito intensivo devido à lotação da unidade. Metade das instituições brasileiras (49,7%) não possuía protocolos de triagem de leitos intensivos instituídos. Conclusão: A recusa de leito pela alta taxa de ocupação é frequente nas unidades de terapia intensiva do Brasil. Ainda assim, metade dos serviços do Brasil não adota protocolos para triagem de leitos.


ABSTRACT Objective: To obtain data on bed refusal in intensive care units in Brazil and to evaluate the use of triage systems by professionals. Methods: A cross-sectional survey. Using the Delphi methodology, a questionnaire was created contemplating the objectives of the study. Physicians and nurses enrolled in the research network of the Associação de Medicina Intensiva Brasileira (AMIBnet) were invited to participate. A web platform (SurveyMonkey®) was used to distribute the questionnaire. The variables in this study were measured in categories and expressed as proportions. The chi-square test or Fisher's exact test was used to verify associations. The significance level was set at 5%. Results: In total, 231 professionals answered the questionnaire, representing all regions of the country. The national intensive care units had an occupancy rate of more than 90% always or frequently for 90.8% of the participants. Among the participants, 84.4% had already refused admitting patients to the intensive care unit due to the capacity of the unit. Half of the Brazilian institutions (49.7%) did not have triage protocols for admission to intensive beds. Conclusions: Bed refusal due to high occupancy rates is common in Brazilian intensive care units. Even so, half of the services in Brazil do not adopt protocols for triage of beds.

La gestión de camas al interior de los centros asistenciales es fundamental para la atención de las necesidades de salud de la población. Actualmente, en Chile se cuenta con escasas herramientas informáticas que agilicen las funciones que realizan las unidades de gestión de camas de los centros asistenciales. El objetivo del presente artículo es describir la implementación de un sistema informático de gestión de camas en tres hospitales de mediana (Modular en La Serena) y alta complejidad (San José del Carmen en Copiapó y San Juan de Dios en La Serena) de la red pública de salud de Chile. El proceso utilizó el de diseminación e implementación, lo que permitió contar con un flujo coherente de gestión de camas, a saber: solicitud, asignación de cama, traslado, hospitalización y egreso de paciente. Asimismo, se identificaron los actores relevantes y las variables mínimas para el adecuado proceso. La implementación del sistema se llevó a cabo en etapas de validación y configuración de la plataforma en cada centro asistencial, capacitaciones a los usuarios y acompañamiento de la puesta en marcha. A la fecha, los tres hospitales cuentan operativamente con el sistema informático de gestión de camas hospitalarias, no reportando dificultades en su uso. El próximo desafío es efectuar una evaluación integral del impacto de la plataforma, utilizando los indicadores acordados con los equipos clínicos/administrativos de los centros de salud.


The management of beds within healthcare centers is essential for meeting the health needs of the population. Currently, in Chile there are few computer tools that streamline the functions performed by the Bed Management Units of healthcare centers. The objective of this article is to describe the implementation of a bed management computer system in three hospitals of medium (Modular-La Serena) and high complexity (San José del Carmen-Copiapó y San Juan de Dios-La Serena) of the Chilean public health network. The process used the Framework of dissemination and implementation, which allowed for a consistent flow of bed management, namely: request, allocation of bed, transfer, hospitalization and patient discharge. Likewise, the relevant actors and the minimum variables for the adequate process were identified. The implementation of the system was carried out in stages of validation and configuration of the platform in each healthcare center, user training and follow-up of the start-up. To date, the three hospitals have an operational computer system for managing hospital beds, reporting no difficulties in its use. The next challenge is to carry out a comprehensive evaluation of the impact of the platform, using the indicators agreed upon with the clinical/administrative teams of the health centers.

Objetivo: relatar desafios enfrentados por enfermeiros na gestão de leitos em uma unidade de internação hospitalar durante a pandemia pela COVID-19. Método: trata-se de estudo descritivo, do tipo relato de experiência. As experiências advêm de enfermeiros que atuam em um hospital público de grande porte, localizado no município de Belo Horizonte-MG. As experiências foram coletadas entre março de 2020 a março de 2022 e organizadas em polos temáticos, fundamentados nos pressupostos da análise de qualidade proposta por Donabedian. Resultados: os desafios enfrentados residem na manutenção do distanciamento entre os leitos, compartilhamento de banheiros, adaptações elétricas, criação de leitos de retaguarda, mudança no perfil dos leitos e na comunicação entre profissionais. Conclusão: os resultados convergem com os desafios previamente encontrados na literatura. Entretanto, apresentam de forma minuciosa e estruturada a realidade de uma unidade de internação, a qual tem potencial para auxiliar em situações de crise, sem previsibilidade e arcabouço científico.(AU)


Objective: to report challenges faced by nurses in managing beds in a hospital inpatient unit during the COVID-19 pandemic. Method: this is a descriptive study, of the experience report type. The experiences come from nurses who work in a large public hospital, located in the city of Belo Horizonte-MG. The experiences were collected between March 2020 and March 2022 and organized into thematic poles, based on the assumptions of the quality analysis proposed by Donabedian. Results: the challenges faced lie in maintaining the distance between beds, sharing bathrooms, electrical adaptations, creating backup beds, changing the profile of beds and communication between professionals. Conclusion: the results converge with the challenges previously found in the literature. However, they present in a detailed and structured way the reality of an inpatient unit, which has the potential to help in crisis situations, without predictability and scientific framework(AU)

Método: se trata de un estudio descriptivo, del tipo relato de experiencia. Las experiencias provienen de enfermeros que actúan en un gran hospital público, ubicado en la ciudad de Belo Horizonte-MG. Las experiencias fueron recolectadas entre marzo de 2020 y marzo de 2022 y organizadas en polos temáticos, a partir de los supuestos del análisis de calidad propuesto por Donabedian. Resultados: los desafíos enfrentados radican en mantener la distancia entre camas, compartir baños, adaptaciones eléctricas, crear camas de respaldo, cambiar el perfil de las camas y la comunicación entre profesionales. Conclusión: los resultados convergen con los desafíos previamente encontrados en la literatura. Sin embargo, presentan de forma detallada y estructurada la realidad de una unidad de hospitalización, que tiene el potencial de ayudar en situaciones de crisis, sin previsibilidad y marco científico.(AU)

Al inicio de la pandemia en México, el Hospital Regional de Alta Especialidad "Dr. Juan Graham Casasús" contaba con 156 camas para la atención de pacientes críticos por sintomatología correspondiente a COVID-19, para abordar esto, se incrementó en un 50% la capacidad hospitalaria durante el periodo crítico. En los primeros 6 meses de pandemia se brindó atención a cerca del 60% de la población de la región que requirió atención especializada por COVID-19 pero a pesar de los resultados se concluye que COVID-19 representa un importante desafío en el sureste de México. Sobre la base de los resultados actuales se evidencia la necesidad de integrar la detección del SARS-CoV-2 de manera rutinaria reconociendo una nueva normalidad en un hospital de tercer nivel en Tabasco, México


At the beginning of the pandemic in Mexico, the "Dr. Juan Garaham Casasús" Hospital had 156 beds for the care of critical patients due to symptoms corresponding to COVID-19. To address this, hospital capacity increased by 50% during the critical period. In the first six months of the pandemic, care was provided to about 60% of the population in the region that required specialized care for COVID-19, still despite the results, it concluded that COVID-19 represents a challenge in the southeast of Mexico and based on the current results, it is evident the need to integrate the detection of SARS-CoV-2 routinely, recognizing a new normal in a tertiary hospital in Tabasco, Mexico.

ABSTRACT

SUMMARY: To describe the physical therapy protocols used in critically ill patients to attenuate skeletal muscle atrophy. We conducted a search in PubMed and Embase from inception to November 2020. Observational or experimental studies published in English or Spanish that evaluated the effect of physical therapy protocols on the attenuation of skeletal muscle atrophy in critically ill patients through muscle strength or mass measurement were considered eligible. Studies were only included if they reported a detailed description of the dosing of the interventions. Seventeen studies met the eligibility criteria. We included randomised clinical trials (n = 16) and observational studies (n = 1). The total population of the included studies was 872 critically ill patients. The studies aimed to evaluate the reliability, safety or effectiveness of neuromuscular electrical stimulation (n = 10) protocols, early mobilisation (n = 3), ergometer training (n = 2), transfers in tilt table (n = 1), and blood flow restriction (n = 1). Physical therapy protocols are part of the critically ill patient's integral management. Strategies such as passive mobilisation, in-bed and out-of-bed transfers, gait training, ergometer training, and neuromuscular electrical stimulation substantially impact critically ill patients' prognoses and quality of life after hospital discharge.


RESUMEN: Describir los protocolos de terapia física usados en pacientes críticos para atenuar la atrofia muscular esquelética. Realizamos una búsqueda en PubMed y Embase desde el inicio hasta noviembre de 2020. Se consideraron los estudios observacionales o experimentales publicados en inglés o español que evaluaron el efecto de los protocolos de terapia física en la atenuación de la atrofia del músculo esquelético en pacientes críticos a través de la medición de la fuerza o la masa muscular. Los estudios solo se incluyeron si informaron una descripción detallada de la dosificación de las intervenciones. Diecisiete estudios cumplieron los criterios de elegibilidad. Se incluyeron ensayos clínicos aleatorizados (n = 16) y estudios observacionales (n = 1). La población total de los estudios incluidos fue de 872 pacientes en estado crítico. Los estudios tuvieron como objetivo evaluar la confiabilidad, seguridad o efectividad de los protocolos de estimulación eléctrica neuromuscular (n = 10), movilización temprana (n = 3), entrenamiento con ergómetro (n = 2), transferencias en mesa basculante (n = 1) y restricción del flujo sanguíneo (n = 1). Los protocolos de terapia física forman parte del manejo integral del paciente crítico. Es- trategias como la movilización pasiva, los traslados dentro y fuera de la cama, el entrenamiento de la marcha, el entrenamiento con ergómetro y la estimulación eléctrica neuromuscular tienen un impacto sustancial en el pronóstico y la calidad de vida de los pacientes críticos después del alta hospitalaria.

ABSTRACT

Frailty is a common elderly syndrome with high prevalence, frailty after longtime bedridding will aggravate the physical frailty.Exercise intervention can ameliorate the physical function of frail patients, enhance their self-care ability and reduce the occurrence of poor prognosis.Due to the special environment of bedridden frail elderly patients, the exercise intervention is different from the traditional exercise program.Considering the physical ability and cognition of bedridden patients, active and passive physical exercise, auxiliary equipment exercise and neuromuscular electrical stimulation are helpful to improve frailty and ameliorate the life quality of the elderly.
